consent forms

Hi i have been asked to give an indian head massage on someone under the age of 16 years, which is fine as i do my own children all the time.
what i would like to know is what so i put in a concent form for her mum to sign???
any help would be great

I would just add a line to the effect that as the parent/guardian of the child they give their consent to the child receiving treatment and understand any contraindications, contra-actions, benefits etc of the treatment and get them to confirm that the information on the consultation form is correct. and obviously a signature and date. That is essentially what I did with my neighbour's son as a case study a couple of years ago and he was 15, fully understanding etc. but I had to do it for legal etc reasons.

I just use my normal consultation form then, when mum (or guardian) signs the form where a client normally signs it, I put (mum/other) in brackets. No problem, nor reason to print off a different form.

That sounds about right.
You wouldn't want any insurance problems...
