need help with my leave navy leave chit?

i need to know what every little box means on it all 33 of them
my supervisor is the one making me write it

Why dont you ask your supervisor, or atleast someone in your chain that can approve your leave?

OK, I'm former Navy and have filled out many leave chits in my day. Most are electronic now. You should have one with in your command intra-net or on a shared drive.

The only major instruction I could find for you is a DoD instruction. I put a link. Most of the other instructions and guidance are local policy. Your major commands will set specifics.

Somehow I get the feeling you made a mistake on not doing your leave right and are given an assignment. Good luck. Look for local instructions.

Without being able to look at the chit itself....no one can tell you what each box means from memory. You need to go through your chain of command or talk to a buddy who can explain it. Each box is pretty self explanatory though. Speak with your higher up...he/she will tell you what each box means. No one on here can help you. Except for telling you to speak directly to your super yourself.
